These fees are non-refundable. Total Fee: $67.00 Passport renewal by mail (customer submits). You may qualify to use this form only if: 1. the issue date of your last passport was less than 15 years ago 2. You were 16 or older when it was issued 3. Your name is the same or you can submit name-change documentation (marriage certificate or court order) 4. You can submit this passport; and 5. It is undamaged ---------- Expedited Service (Form DS-11 or DS-82) Add $60 per application and 2-way overnight delivery Expedite fee does not include overnight delivery. Customer must arrange for overnight delivery to ensure receipt within 3 to 4 weeks. ---------- If international departure in less than two weeks, or passport is needed for foreign visas, application at a Regional Passport Agency may be advised.
